Quality HealthCare Consulting, LLC (QHC) facilitates transformation for practices seeking recognition as a Patient Centered Medical Home (PCMH). Our company utilizes Basecamp, an innovative cloud-based project management program that tracks and monitors tasks. Additionally, the software incorporates National Committee for Quality Assurance (NCQA) approved Level III examples and QHC instructions. QHC provides an NCQA PCMH Certified Content Expert who will serve as a Practice Transformation Coach. This coach will guide the practice team through the necessary workflow steps within Basecamp, review data and policies submitted by the practice team, make suggestions for workflow changes, and lead a weekly call to help ensure that the practice achieves the highest possible level of recognition for PCMH from NCQA. Furthermore, this coach will be available whenever needed via phone or email to answer any of the practice team’s questions.

Each consultation typically takes 12 months to complete. However, this timeframe may vary depending on the size, resources, and capabilities of the group. The process begins with an onsite assessment and an agreement for at least 10 billable hours per month. More may be needed at the discretion of the client.

Rates are set per hour, and clients are responsible for travel expenses.

How can transforming to a PCMH work for you?

As of June 2013, 5730 practices have received NCQA’s PCMH recognition. ERHS is currently in the process of achieving recognition.

As a recognized PCMH provider, goals will include improved medication adherence, decreased ER and hospital utilization, improved self management education, improved patient satisfaction and an overall improvement in clinical outcomes.

PCMH also encourages patients to become more actively involved in their health while allowing everyone on the care team to work to the highest level of their license.
